In 2018-2019, at the Keck School of Medicine of the University of Southern California, we developed and piloted a narrative-based health systems science intervention for patients living with HIV and medical students in which medical students co-wrote patients’ life narratives for inclusion in the electronic health record. The pilot study aimed to assess the acceptability of the “life narrative protocol” from multiple stakeholder positions and characterize participants’ experiences of the clinical and pedagogical implications of the LNP. Students were recruited from (...) KSOM. Patients and staff were recruited from the Maternal, Child, and Adolescent/Adult Center for Infectious Disease and Virology at Los Angeles County+USC Medical Center. Ten patients, seventeen students, and ten MCA staff participated in the pilot study. Qualitative methods were used to gather data from students’, patients’, and staff’s perspectives. Three themes emerged from the thematic analysis: patients’ life narratives conveyed their unique life experiences and voices; the protocol could result in wide-ranging effects on HIV care; the LNP enabled students to contribute value to patients’ healthcare. Across groups, participants considered the LNP an acceptable intervention. The LNP, its limitations, and implications for HIV care, narrative medicine, and health information technology are presented. (shrink)

Background Residual dried blood spots (rDBS) from newborn screening programmes represent a valuable resource for medical research, from basic sciences, through clinical to public health. In Hong Kong, there is no legislation for biobanking. Parents’ view on the retention and use of residual newborn blood samples could be cultural-specific and is important to consider for biobanking of rDBS. Objective To study the views and concerns on long-term storage and secondary use of rDBS from newborn screening programmes among Hong Kong Chinese (...) parents. Methods A mixed-method approach was used to study the views and concerns on long-term storage and secondary use of rDBS from newborn screening programmes among Hong Kong Chinese parents of children 0–3 years or expecting parents through focus groups (8 groups; 33 participants) and a survey (n = 1012, 85% mothers) designed with insights obtained from the focus groups. We used framework analysis to summarise the themes as supportive factors, concerns and critical arguments for retention and secondary use of rDBS from focus group discussion. We used multiple logistic regression to assess factors associated with support for retention and secondary use of rDBS in the survey. Results Both in focus groups and survey, majority of parents were not aware of the potential secondary use of rDBS. Overall secondary use of rDBS in medical research was well accepted by a large proportion of Hong Kong parents, even if all potential future research could not be specified in a broad consent. However parents were concerned about potential risks of biobanking rDBS including leaking of data and mis-use of genetic information. Parents wanted to be asked for permission before rDBS are stored and mainly did not accept an “opt-out” approach. The survey showed that parents born in mainland China, compared to Hong Kong born parents, had lower awareness of newborn screening but higher support in biobanking rDBS. Higher education was associated with support in rDBS biobanking only among fathers. Conclusion Long-term storage and secondary use of rDBS from newborn screening for biomedical research and a broad consent for biobanking of rDBS are generally acceptable to Hong Kong parents given their autonomy is respected and their privacy is protected, highlighting the importance of an accountable governance and a transparent access policy for rDBS biobanks. Tennenbaum's Theorem yields an elegant characterisation of the standard model of arithmetic. Several authors have recently claimed that this result has important philosophical consequences: in particular, it offers us a way of responding to model-theoretic worries about how we manage to grasp the standard model. We disagree. If there ever was such a problem about how we come to grasp the standard model, then Tennenbaum's Theorem does not help. This study attempts to highlight and clarify the central themes in four important theoretical perspectives on the nature of war and the international order. Perhaps because it is an enlarged version of the Wiles lectures, the book is written with a refreshing verve and fluent, relaxed elegance—purchased at the cost of some redundancy and lack of crispness. The bulk of the work consists of four largely independent essays, each dealing with a single thinker or outlook. The essay dealing with (...) Marx and Engels is the most helpful and rich: by focusing on Engels’s voluminous but hitherto neglected military writings, Gallie is able to shed intriguing new light on the revolutionary strategy of the founders of Marxism. It is perhaps not surprising that the most ambitious chapter theoretically, the one on Kant, is the least satisfactory. The author offers a sensible corrective to many misreadings of Kant’s political works and presents a fairly accurate overview of Kant’s internationalist program. But because Gallie fails to take into account either the crucial Kantian concept of the State of Nature or the basic Kantian distinction between virtue and justice, the real difficulties tend to be glossed over, and Kant is made to seem rather less hardheaded, and more moralistic, than he in fact is. Bioethical issues are perplexing, profound, and politically divisive. The Lanson Lectures in Bioethics Series was founded in the belief that philosophical elucidation can clarify the nature of these difficult issues, and can lead to their resolution. The present volume collects the first five previously unpublished lectures delivered by five preeminent moral philosophers between 2016 and 2022. Drawing a distinction between two concepts of dignity, Jonathan Glover brings the distinction to bear on the issues of assisted suicide, embryo research, and genetic (...) choices. F. M. Kamm argues that doctors are morally permitted to intentionally cause death, or assist in its being intentionally caused, when either death is imminent anyway and intentionally causing it can alone stop the pain, or if the patient has already decided—not unreasonably—that death is his least bad option. On the question whether smokers who contract lung cancer are entitled to state-supported healthcare, T. M. Scanlon argues that the reasons that individuals have for wanting to engage in risky activities need to be compared with the relevant costs society has to bear to provide healthcare for those who suffer illness or injury as a result of these activities. Contrary to P. F. Strawson’s view that a psychiatrist can only “treat” an insane patient, Victor Tadros argues that it is often right to reason with, and communicate to, (nonresponsible) mentally ill persons. Peter Singer discusses questions pertinent to pandemics: How to distribute scarce medical resources and vaccines ethically? Whether to relax the standard for volunteers willing to participate in vaccines research? How to compare the trade-off between saving lives and saving the economy regarding lockdowns? How to prevent pandemics in future? These lectures are followed by critical commentaries by Alastair Campbell, Chun-Yan Tse, Peter Chau, Derek Baker, and Alexandre Erler respectively. Each commentary (except the inaugural lecture) is followed by a rejoinder. Holy Road: The Beat Movement as Spiritual Protest” (1991), the Beats and their pilgrimages have most emphatically not been taken seriously. Both critics and scholars have been unkind to members of the Beat generation, and this unkindness has manifested itself in relative neglect of the Beats in all possible aspects of scholarly investigation. But nowhere does this appear more pronounced than in the area of Beat religion. Though some recent scholarship has been picking up from this earlier neglect and it is now [End Page 165] possible to find serious biographies of most of the major Beat writers and some belated attention to the movement as a literary phenomenon, the only book-length treatment of Beat Buddhism I was able to locate (Tonkinson 1995) hardly qualifies as a scholarly work and was published not by an academic press but as a special feature of the Buddhist magazine Tricycle. Prothero hypothesizes that this is because of the movement’s reputation as rebellious and antinomian—in its heyday the movement was characterized in identical unsavory terms by publications representing interests as diverse as Life and Playboy, with the latter describing the Beats as “nihilists” and the former claiming that the Beats were at war with “Mom, Dad, Politics, Marriage, the Savings Bank, Organized Religion, Literary Elegance, Law, the Ivy League Suit and Higher Education, to say nothing of the Automatic Dishwasher, the Cellophane-wrapped Soda Cracker, the Split-Level House and the clean, or peace-provoking H-bomb” (Prothero 1991: 206). Norman Podhoretz of Partisan Review went further still, claiming that the Beats were “hostile to civilization” and that the movement as a whole amounted to “a revolt of the spiritually underprivileged and crippled of soul” and “an affirmation of death” (ibid., 206). One couldn’t ask for a more passionate disavowal of the Beats or a more thorough polemic that the movement was profoundly antireligious.This could not contrast more with the Beats’ representation of their movement and its aims, with Jack Kerouac going so far as to reinterpret the crucial term “Beat” (a term for someone down and out originally borrowed from criminal street jive) as a noun derived from the adjective “beatific,” a specifically religious term familiar from such phrases as “the beatific vision” (ibid., 206–207). Several of the representative Beat writers were practicing Buddhists, influencing the entire movement to such an extent that Buddhist ideas and imagery suffused Beat literature as a whole. Ginsberg and Kerouac especially emphasized the positive and constructive elements of their spiritual vision, arguing that literature such as On the Road and Howl should be understood as religious affirmations of what is good and holy rather than as purely critical undertakings: “For the crucifix I speak out, for the Star of Israel I speak out, for the divinest man who ever lived who was German (Bach) I speak out, for sweet Mohammed I speak out, for Buddha I speak out, for Lao-tse and Chuang-tse I speak out” (ibid., 206–207). Or, as Allen Ginsberg put it less lyrically (and rather more bluntly): “Howl is an ‘Affirmation’ by individual experience of God, sex, drugs, and absurdity... the poems are religious and I meant them to be” (ibid., 207).But for the most part, Beat asseverations in no way diminished scholarly wariness about the Beat movement and its spirituality—as Prothero concludes, “the tendency among literary scholars is to see these [religious] concerns as tangential rather than constitutive” while “historians of American religion who have explored beat spirituality have tended to focus almost exclusively on the beats’ engagement with Zen and then to dismiss that engagement as haphazard” (ibid., 207).It is my contention that the mainstream scholarly neglect of Beat spirituality is in some part... We define ethical system infrastructure as being composed of three major factors – means, motivation, and opportunity. Means are defined as organizational rules, policies, and procedures. Motivation focuses upon the values and the interests being pursued by the position occupant and the organizational value system, while opportunity is discussed in terms of the environment in which the dilemma occurs, proposing that position in the hierarchy presents its own unique set of ethical dilemmas. Ethical breeches are discussed in terms of the (...) interactional processes among means, motivation, and opportunity. They are for him heterogeneous but the lower order leads to the higher. Without God, man is misery and darkness. With God, man can do all things (p. 118). The third part (pp. 115-219) is entitled The Christian Philosopher. It shows that man is made for the infinite. The true call of humanity is religious (p. 206). This small volume is the first in a new series devoted to bringing to light various figures and aspects of seventeenth-century thought that have either been set aside, forgotten, or are only dimly known. Louis Thomassin (as well as Bernard Lamy who is the subject of the second volume that has appeared in this series) certainly falls in these categories. Although his name often turns up in discussions of Cartesianlsm, Jansenism, Malebranchism, etc., very little consideration is ever given to him as a fairly important personage of the time, or to his ideas and the role they played. This volume by Dr. Clair will be of much use to seventeenth-century scholars in providing the basic data about his life, his connections with several of the vital movements of the period, the fundamental bibliographical materials, as well as publishing two texts and twenty letters not before available. The biographical section (pp. 1-84) traces (a bit too episodically) Thomassin's education, his career among the Oratorians as a professor of humanities, philosophy, and theology, his possible involvements with the Jansenists, his difficulties as a result of his first published work, Dissertations sur les Conciles (1667) (suppressed the same year), his later successes for his many other works in theology, philosophy, history, and pedagogy, and his relations with various major and minor figures of the mid- and late seventeenth century, like Arnauld, Malebranche, Richard Simon, Bayle, Huet, etc. Section II (pp. 85-99) gives a detailed bibliography of Thomassin's published works, plus a list of his manuscripts located in various French libraries. It cannot be denied that a correct notion of justice is very relevant to the solution of most modern problems. The constant appeal to the rights of individuals and groups of individuals and the widespread practice of injustice bear witness to this. Yet the very frequency with which the terms justice and injustice are used has, one fears, reduced those words to mere labels for whatever claim it is desired to make or deny, without regard for the existence of an (...) objectively just order in public and private life. There is need then of a reaffirmation of the idea of justice and an application of its principles, in terms suitable to our modern modes of thought, to current problems of human relationships.
